Like many young Japanese women, Tomoko (MAHO WASADA) is conservative and desperately needs a job. A recent graduate of the best film acting school in Japan -- Film Center Tokyo -- the pretty Tomoko auditions for a role in a mental hospital training video.
However, the charming medical director, Dr. Kawai (KOJI OKUDA) admits the truth: what he really needs is a spy -- an actress to pose as mentally ill and eavesdrop on patients. Saddled by debt incurred by her gambling-addicted Aunt Maya (SHIKA MACKENZIE), Tomoko agrees to take on this bizarre assignment and go undercover.
She enters the world of the mentally ill and soon discovers that this hospital is anything but ordinary...
She meets Dr. Akita (YOSHIKO TANAKA) the clinical psychologist with a sunshine attitude who specializes in baffling mental patients with flash card tests. Tomoko wonders if Dr. Akita is truly helping patients, or is she trying to brainwash them?
Among the Japanese patients is one foreigner, the grizzled American Eddie Burke (MICHAEL MADSEN). Filled with conspiracy theories, Eddie tells Tomoko that this hospital -- and the entire culture in which she lives -- is dominated by a powerful, secret group of people whose mission is nationwide mind control.
Tomoko tells Eddie, "You are completely nuts." However Tomoko soon meets other patients who confirm Eddie's story:
Danny (TERUO KOZU) an idiot savante who could unlock the secret... Tetsuya (AUSTIN UCHINO) a former kickboxer and defender of the weak... and Suzi (HORI SAYAKA) the sexiest patient in the hospital who knows it all.
Eventually Tomoko begins to side with the patients, who all fear Nurse Ishida (ASAYOSHI FUJIWARA), the cruel enforcer of hospital rules who enjoys torturing -- and killing -- patients for sport.
Coming full circle, Tomoko sheds her traditional values and evolves into a brave, free-thinking individual. With time running out, Tomoko is the only person who can save the people of Japan. In the end, she must play a sadistic game of sexual subterfuge in a mental hospital known as...
